Service Coordination

A specially trained person called a Service Coordinator provides assistance with life planning and links you to services and supports that fit your life. The Service Coordinator works with you and the important people in your life, including family, friends, and chosen professionals to develop an Individual Service Plan. This group of people forms your “Circle of Support”. The Service Coordinator helps you to live out the life you have planned through securing residential, vocational, educational, health, and community supports to fit your plan. The Service Coordinator also provides expert help in accessing various community resources including Social Security, Medicaid, Department of Labor, and many other services. The Service Coordinator meets with you regularly to assure success and satisfaction with your plan. As your life unfolds, your Service Coordinator will be there to help you with the ongoing planning you will want to do to keep your life moving toward your goals.

Individualized Residential Alternative

This service, which offers small homes and apartment living, is designed to assist you in living as independently as possible in your community. While each home varies in size, location and style, they are designed and decorated by the people who live there to meet their needs, desires and preferences.

While living in your home, you will be a full partner with your Service Coordinator and Rivershore staff to develop an Individualized Service Plan. This plan will identify supports and services that are tailored to help you achieve your life dreams. Full time staff involvement and flexible access to supports and services including nursing, psychological services and coaching in daily living skills combine to assure this service focuses on you as a person.

This living option provides the opportunity to participate in the care of your own home, and to enjoy your community to the fullest.

Individualized Service Environment

ISE services are designed for people who live in their own apartment or house, or in a family dwelling. ISE services support you with increasing your independence in specific areas, allowing you to be successful in your chosen living arrangement. Services are available to all age ranges.

An individualized Residential Habilitation Plan of Service is developed with you based on your needs, personal goals and interests. This plan is developed with you, your Service Coordinator, family or friends and the ISE staff. Highly trained staff will visit your home and provide supports to help you achieve your goals, which are specific and individualized to you. These supports include assisting you in completing all necessary daily activities, assisting you with attending any needed medical appointments, and gaining further independence, productivity and inclusion in your community.

This service also provides flexible scheduling of staff to support your desired work, school, or activity schedules.

Guest Respite Services

Rivershore is pleased to offer the only Guest Respite in Niagara County, which provides customized services and supports to adults 18 years or older. Guest Respite Services are available 24 hours a day, 7 days a week.

Rivershore’s Guest Respite Services are provided within an easily accessible residence in Lewiston, NY. The residence provides a private bedroom, which allows for personal privacy, but still provides opportunities for social interaction and leisure activities with the residents of the home.

Rivershore’s Guest Respite Service provides needed relief for both the caregiver and the person enjoying their respite away from home.

Transition

Moving through adolescence into adulthood is a major transition in everyone’s life. Planning for life following our high school years can be challenging and filled with uncertainty. Through Service Coordination, Transition Planning can support you through this important time and help get your adult life off to a good start.
